Mobile Whānau Ora – Chronic Disease

Overview

This is a mobile service providing individualized nursing assessment and wellness-orientated planning, to make positive health gains in the Hauraki area. Working with clients in their own homes, a broader and better understanding is gained of some of the implications and challenges that they may face.

With three Registered Nurses working alongside two Community Health workers, the team provides:

  • Education
  • Life-style programmes
  • Co-ordination of health services

The purpose of the service is to empower and educate clients and their whānau/family, to understand and manage their chronic disease.

Delivered in a holistic way with cultural appropriateness and sensitivity to meet the needs of each client, the service provision is for clients of all cultures over the age of 25 years, who have a chronic disease e.g:

  • Heart Disease
  • Diabetes
  • Respiratory Disease
     

Working predominantly with Māori clients, the service also targets clients who smoke.

The mobile service makes the management of chronic disease accessible to clients who are not able to come into a Te Korowai clinic.

Referral Process

Referrals come to the service through:

  • GPs
  • Other health professionals
  • Self-referrals
